You have been scheduled for an appointment in our office.  We realize that going to see a specialist can be a trying experience.  In order to make your visit as pleasant as possible, we are asking you to browse this website, which contains information about our practice and staff.

If you have never been seen in our office before, or if you have not been seen for the past year, we need some information for our files.  Please print the medical questionnaire (six pages) and patient registration, complete both forms, being sure to fill in all blanks, and sign everywhere requested.  Bring the completed forms with you on the day of your office visit.  Also bring your health insurance cards, as we will need to make a copy for our records.  In addition, please remember to bring all the  medications you are presently taking, in their original bottles.

You should also bring all x-rays and test results relating to your problem. This means actual x-ray, CT, or MRI films, in addition to the reports and EMG reports.
