air embolism
There are a number of "noncollapsible" veins in the brain, and in particular the superior sagittal sinus, the sigmoid and transverse sinus, and the cavernous sinus.  During an operation on the brain, air may potentially get into these sinuses, and it can then become trapped in the right atrium.  This could potentially result in death.
